Fillable Membership Application

Below is the link to the 2025 Membership Application/Renewal form.  

Please fill it out, sign it, print it and return with your dues. Dues are $30 for a single member or $40 for a couple.  

Once you complete and sign the form, you can email it to Angie Jones, Membership Director (adjones61@gmail.com) and let her know how you will be paying (see options below).  

Other options include bringing the completed signed form and a check or cash to a meeting or event and giving them to one of the Club Officers (see https://wwmaf.com/about-us-%26-officers).  They will forward your form and payment to the Membership Director. 

Your membership will be acknowledged by an email from the Membership Director as quickly as possible.
